This Request for Proposal (RFP) is an invitation to Pacific health providers for proposals under Workstream A (Pacific health providers), proposals for new work projects, initiatives and models of care provided by Pacific health providers to Pacific communities or in collaboration with Pacific community organisations. The objective of the fund is to support:
• innovative Pacific models of care that support community action and strengths-based approaches that lead to Pacific communities having greater control over decisions concerning their health and wellbeing.
• innovative Pacific approaches that are shown to be effective and that can clearly demonstrate contributions to improving the health and wellbeing outcomes for Pacific peoples.
A key feature of the Fund is its intention of promoting permanent improvement in health outcomes for Pacific people. This means that innovations should not just be one-off demonstrations of ideas but be supported by independent evaluation that verifies the benefits delivered and provides a sound basis for promoting wider implementation.
