This Request for Proposal (RFP) is an invitation to New Zealand suppliers to submit a proposal for the ‘Community referral options for weight management support’ contract opportunity. The aim of the project is to facilitate PHO and primary care providers to work with an existing commercial or community weight management provider to develop new referral pathways and a suitable existing community programme to refer people at high risk of developing diabetes and cardiovascular disease to reduce their weight. The weight management provider should be one that is not traditionally utilised or referred to by primary care. The service is to be delivered in the Wellington region.
