West Coast District Health Board (WCDHB) requires a Community Care Technology Platform (a Primary Care system which includes the capture and management of Tertiary Care Ambulatory Out Patients patient data too).
The system must also include staff scheduling. It must support Home-Based Support workers as part of the patient’s circle of care; and include application of staff travel policies for home based care (Ministry of Health).
We need a system that is digitised, standardised, measurable and continuously improved. It must be an off-the-shelf system with minimal work to implement and integrate. It can be made up of several systems but has to look like one system.
