Overview
E Tipu E Rea Coordination Hub Services
To develop and implement a coordination hub service within the E Tipu E Rea framework, a service which provides a single point that concerned clinicians and other entities can refer to, for a woman/child with needs beyond that which the clinician/provider can immediately address; overcomes the current issue of clinicians/providers not knowing what support is available and how to access it; a ‘hub’ which receives the referral, triages, and passes on to the most appropriate service; undertakes tracking of referrals, volumes and the management and administration of the Community Options fund.
