Overview

This Request for Proposal (RFP) is issued by the Hutt Valley District Health Board (HVDHB) on behalf of HVDHB and Capital and Coast District Health Board (CCDHB) - collectively called the District Health Boards (the DHBs).

The purpose of this RFP is to invite detailed responses for the delivery of all aspects of long term home and community support services (the Services) for older persons for the DHBs. The DHBs are established under the New Zealand Public Health and Disability Act 2000 and are Crown agents under the Crown Entities Act 2004.

In June and July 2018, the Boards of the 2 DHBs confirmed their agreement to proceed with re-tendering for the provision of HCSS across the 2 DHBs on a bulk-funded basis with the aim of securing two or more HCSS providers.

The Services sought in this tender are for long term services to meet the needs of complex and non complex clients for people over the age of 65 and other people with disabilities who fit funding stream criteria. The service requirements incorporate an enabling model of care in which older people are supported to maintain independence, to care for themselves, to be connected with family and whanau and to participate in their communities for as long as possible.

The HCSS we wish to purchase include: assessment, service (support) planning, and coordination of support workers to deliver the support plan. It includes supportive personal care and health related household management, allied health intervention, service outcome evaluation, client review and safe discharge from HCSS provision, with the capability to provide quality services to our clients.
