The DHB Interim Care Scheme from here on in known as the ICS involves admission of patients with non-acute injuries or conditions into an ARRC facility with Residential Facility-Hospital Level Care certification. The patient cohort will likely be medical or surgical patients, non-weight bearing, and awaiting, or have undergone, surgical treatment or intensive rehabilitation. This cohort of patients will be unable to safely function at home with a package of care. The purpose of the ICS is to provide a time-limited period for patient recovery, based on an individual short-term care plan, to maximise the person’s level of rehabilitation and maintenance of functioning during the period of care, and to enable them to safely return home or back to their previous community setting, or return to hospital for further rehabilitation
