The purpose of this RFP is to contract for the delivery of an in-home strength and balance falls prevention programme to Waitemata DHB. The purpose of the in-home programme is to fill the current service gap of high risk people requiring a tailored strength and balance programme delivered within their home. The contract awarded as a result of this RFP will be a terminate 30 June 2019. Monitoring and evaluation of the Service and the other falls prevention services will be undertaken to determine if investment should continue after 30 June 2019. The Service is bulk funded. As the budget for the service is fixed, a price response to this RFP is not required and price will not form part of the RFP evaluation. Responses will be evaluated on the quantity and quality of the Services that can be provided within that budget.
