Overview

Canterbury DHB is seeking a community based provider for the on-going management of the Advance Care Planning (ACP) Coordination Service. The service is delivered across the Canterbury DHB area by 1.6 FTE Clinical Facilitators and 0.6 FTE Administrators.
Advance Care Planning (ACP) is a process of discussion and shared planning for future health care which involves the individual, their family/whānau (if the person chooses) and their health care professionals. Ideally these discussions are formalised in an Advance Care Plan (for competent patients) or Medical Care Guidance (MCG) document (for non-competent patients) which is accessible to all current and future health care providers, and to family/whānau members. The plan is a legally valid document; a clear statement of the person’s wishes, preferences, values, and goals relevant to all current and future health care. ACP may be used to inform decision-making after loss of capacity. Plans are electronically published after a clinical review, this ensures that Health Professionals throughout Canterbury can access the plan and where the person requires care and is unable to speak for themselves the plan content meets quality requirements and is legally and medically valid.
