Southern District Health Board (SDHB) and Well South Primary Health Network (WSPHN) are seeking assistance to lead the development of a Primary and Community Strategy and Action Plan. The Strategy should provide a clear model of how primary and community health services will be configured and integrated with secondary/tertiary care to deliver timely, effective and sustainable services for the Southern DHB population in 2030.
